This case report describes the treatment of a 20-year-old man with hemifacial microsomia, facial asymmetry, and a skeletal Class II malocclusion. The results of clinical and radiographic examination indicated that he had Pruzansky-Kaban Type IIb right hemifacial microsomia with abnormal size, shape and function of temporomandibular joint (TMJ) and ramus. Combined orthodontic treatment using orthodontic mini implant and mandibular surgery with unilateral total TMJ replacement were performed to improve the esthetics of the facial profile and function of the stomatognathic system. Treatment goals such as good facial symmetry within normal limits, functional jaw movement, good occlusion, and a satisfied patient were achieved. Total TMJ replacement with orthognathic surgery for TMJ and jaw reconstruction is a valid option for nongrowing patients with hemifacial microsomia.
